Case 2023: Diego (expert opinion by B. Konety)

Diego, 76 years old, plays flamenco guitar in several local bars. When he performs it always ends up in a flamenco dance fest. Once he tried to join the dancers, which was hilarious because of his two “left feet”. Everyone agreed that his place is behind his guitar, rather than the dancefloor.

He recently underwent cystoscopy for painless haematuria, which revealed a papillary tumour.

Assessment summary:

  • Diagnostic transurethral resection of bladder tumour (TURBT), pathological review:
    • Minimal pT2 UCa
    • Unifocal, no concomitant carcinoma in situ (CIS)
  • CT imaging: no upper tract disease and bladder mass suggestive for cT2b cN0 cM0
  • Ultrasound: no hydronephrosis
  • eGFR: 66 ml/min
  • No peripheral neuropathy
  • No hearing impairment
  • ECOG PS: 1
  • Cardiac ejection fraction: 70%

He received 4 cycles neoadjuvant gemcitabine + cisplatin. RC is planned for this patient.

Which surgical option would you choose for Diego (not taking into account regulatory approval and local restrictions)?
